If you have had a consult with a health professional and he/she has
prescribed you practitioner-only products, then you can purchase those
prescribed products at FourBody. A health professional includes medical
practitioners, psychologists, dentists, pharmacists, physiotherapists,
dieticians, nurses, herbalists, homeopathic practitioners, chiropractors,
naturopaths, nutritionists, practitioners of traditional Chinese medicine,
podiatrists or osteopaths registered under a law of a State or Territory.
All you need to do is fill out the below form with the necessary details.
Once we have received your practitioner's details, a code will
be e-mailed to you within 1-2 business days. This code will give you access to the
practitioner products.
