TECHNICAL INFORMATION
Adenosine plays a unique role in the body as a regulator of cellular metabolism.
It is a metabolite of all cells and is involved in key pathways such as
purinergic nucleic acid synthesis, amino acid metabolism and modulation of
cellular metabolic status.[5]
Adenosine can raise the cellular level of AMP, ADP and ATP
which are the energy intermediates of the cell. It can stimulate or down
regulate the activity of adenylate cyclase and hence regulate the cAMP levels.
cAMP is important in the second messenger system involved in the regulation of
cell activity. Adenosine's major role is to act as a protective injury auto-coid.
In any condition in which ischaemia, low oxygen tension or trauma occurs,
adenosine plays a role. Defects in synthesis, release, action and/or degradation
of adenosine may contribute to the overactivity of the brain excitatory amino
acid neurotransmitters and hence various pathological states.
Manipulation of adenosine receptors influences sleep and
arousal, cognition and memory, neuronal damage and degeneration and neuronal
maturation. Adenosine might have therapeutic implications for neurodegenerative
diseases such as Parkinson's disease, Alzheimer's disease and other neurological
situations such as epilepsy and idiopathic pain.[6]
Epilepsy
It is widely accepted that adenosine is an inhibitory modulator in the CNS,
acting via the inhibitory A1 receptors. Adenosine exerts potent depressant
effects on neuronal firing and synaptic transmission. Activation of adenosine
receptors may therefore be effective in the support of epilepsy.
Excitotoxicity
It is well established that in the CNS, endogenous adenosine plays a pivotal
role in neurodegeneration. A low concentration of adenosine is normally present
in the extracellular fluid, but it increases dramatically during enhanced nerve
activity, hypoxia or ischemia. In these pathological conditions, adenosinergic
transmission-potentiating agents offer protection against ischemic or
excitotoxic neuronal damage. Activation of the adenosine A1 receptor is known to
mediate neuroprotection, mostly by the blockade of Ca2+ influx, which results in
the inhibition of glutamate release and reduction of its excitatory effects at a
postsynaptic level.
Insomnia
Adenosine is accumulated during the waking state as a result of neuronal
metabolism and this in turn is related to drowsiness. Sleep episodes lower the
levels of this substance in the brain. Adenosine receptor antagonists increase
wakefulness (e.g. caffeine), while the agonists promote slow-wave sleep.
Adenosine from the hypothalamus performs functions involving the regulation of
sleep and wakefulness. Understanding these two systems can have repercussions on
clinical problems such as insomnia, hypersomnia and other neuropsychiatric
disorders.
