TECHNICAL INFORMATION
Pancreatic Insufficiency
Pancreatin therapy is used when exocrine pancreatic function is inadequate. To be efficacious, replacement therapy needs to supply adequate quantities of the deficient enzymes to the right place, at the right time and with a favourable pH. The main problems are those of delivery, timing and pH.

a) Lipases: The Lipase enzyme is responsible for the digestion of fats and is highly sensitive to pH and an adequate amount is needed to prevent fat malabsorption. Even with increasing dosage, steatorrhoea may not be avoided due to pH inactivation of lipase.[4]

b) Timing of supplementation: Pancreatin needs to be taken after the meal to maximize the pH-buffering effect of food.

c) Gastric acid suppression: H2-antagonists or proton pump inhibitors are often prescribed with pancreatin. The increased small bowel pH attained improves the survival of lipase enzymes, reduces the duodenal volume flow and prevents the precipitation of bile acids.

Animal pancreatin vs Vegetarian pancreatin
Porcine pancreatin has higher lipase activity than vegetarian pancreatin. This may be more beneficial for those with problems with lipid digestion. Porcine lipase has been the therapeutic standard for several decades and will continue to be the treatment of choice in pancreatic exocrine insufficiency.

Allergies
It has been demonstrated that pancreatic enzymes may have a beneficial effect on expression of intestinal allergic inflammation. Application of pancreatic enzymes has been found to significantly reduce clinical symptoms.

The pineapple protease, bromelain, selectively inhibits the biosynthesis of proinflammatory prostaglandins, apparently by indirect action. It has been suggested that bromelain acts on the prostaglandin synthetic pathway at a site different from that affected by the non-steroidal anti-inflammatory drugs. It is postulated that bromelain acts further down the arachidonate cascade at the thromboxane synthetase step. Circumstantial evidence suggests that bromelain inhibits the synthesis of the "proinflammatory" prostaglandins without affecting that of the "anti-inflammatory" ones.
